YOW includes outreach, short-term support, referrals, and consultation that utilizes evidence-informed outreach models based on the Stages of Change theory. This service model is a method of categorizing and supporting an individual’s readiness for change. The emphasis is on bridging youth to AHT services and other external programs, providing accompaniments and support to ensure successful connections to programs, workers and other service providers.
 
To Book an Appointment:

New clients can self-refer to the program for intake and no appointment is necessary. Intake is done at our Gerrard Street location, Monday to Friday, 10 a.m-12:30 p.m. and 1:00 p.m. – 3:30 p.m. For existing clients, please contact our Gerrard Street location and speak to Youth Outreach Coordinator. AHT providers can also schedule appointments for clients.
